5/21/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She indicated to QMHP-C that she was good. QMHP-C discussed with Ashiyah that this session was going to focus on her goals. 2. Behavior: Ashiyah actively participated in the session by cooperating throughout the session. She verbalized to QMHP-C at times she tries to get along with her peers, but they seem to want to try her all the time. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C reviewed and demonstrated calming techniques with Ashiyah. QMHP-C discussed instances in which calming techniques should be implemented. QMHP-C engaged Ashiyah in a discussion allowing her to discuss with QMHP-C how she has implemented such calming techniques in the home. QMHP-C engaged Ashiyah in a discussion about Ashiyah’s use of self-talk. 2. Intervention: QMHP-C engaged Ashiyah in a discussion on how she has employed self-talk to mediate angry feelings. QMHP-C discussed depressed moods. QMHP-C asked probing questions to allow Ashiyah the opportunity to discuss how she feels when she is depressed. Ashiyah was provided with several age appropriate activities that she may engage in with her family members to promote positive relationships with the family members. QMHP-C identified and listed several positive attributes of Ashiyah throughout the session. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah continues to have low self-esteem as evidenced by her lack of social skills such as maintaining adequate eye contact and talking in inaudible voice tones. Ashiyah admitted to QMHP-C that she struggles with not being able to control her anger because that is just the type of person that she is. 2. Response: Ashiyah discussed with QMHP-C that her mother is the cause of her anger at times when she does not allow her to do certain things. Ashiyah provided QMHP-C with an example such as, “During the whole time that were out, my mother did not allow me to come outside or spend time with my friends, the only thing that I was able to do was go with you Ms. Vaughn”. Ashiyah indicated to QMHP-C, “I only get stressed and depressed when my mom treat me like I am a toddler or something”.
